We need your help!
We would like to offer different sponsorship opportunities for businesses or individuals to participate. Your contribution to this event will go to the Robert A. Kozminski Scholarship & the C.O.P.S. Foundation - West Michingan
There are multiple levels of Sponsorship that you can choose with different perks.
Title Sponsorship ($5000) (1 available) Includes:

In Person: Fill out & Print this form and bring it in to pay in person! If you do not have the ability to print we will have the form available for you to fill out.
By Mail: Fill out & Print this form and print to send by mail with your check payable to West Grand Neighborhood Organization (put in the memo "Golf 4 Koz Sponsorship - Level")
Invoicing: If you need an invoice, please contact West Grand Neighborhood Organization at 616-451-0150 or mail@westgrand.org and we can email or fax it to you.
